If your insurance is an HMO which requires a referral, you will need to obtain the proper authorization and referral prior to your appointment. It is your responsibility to obtain this referral and bring it with you when you come to the office for your appointment. If you do not have your referral, you will not be seen unless your visit is an emergency.

If we do not participate with your insurance, you will be given a receipt that you may submit for reimbursement. We will make every effort to help you submit your claim, however, payment is expected at the time of your examination. If you have any questions concerning your coverage, contact us and we will be happy to assist you prior to your examination.

Co-pays are due at the time of the appointment and are expected to be paid when checking out after your visit. For your convenience, Children’s Eye Care accepts Visa and Master Card as well as personal checks.
