Office
information:
Our office hours are from 8:30 a.m. to 4:30 p.m.,
Monday through Friday. When you arrive for your
appointment, you will be greeted by one of our helpful
professional staff and be asked to fill out a patient
information form. You will be presented with our medical
information privacy notice and be asked to read and sign
it. You will also be asked to present an insurance card,
a driver’s license and social security number for you or
the patient.
We recommend that you call 2 - 3 months ahead to make
a routine appointment. Emergency or urgent cases will be
seen as soon as possible.
You should plan to be in our office for 2 hours for
all initial and annual complete eye examination
appointments. These appointments require the eyes to be
dilated and time is needed for the drops to work
effectively.
